Conservation Incentive Application

Ohop Mutual will review your rebate application and determine if the installed products qualify for the rebate. By checking the box you acknowledge that: the Heat Pump Water Heater purchased is ENERGY STAR ® qualified and is installed at a residence in Ohop Mutual Light Company service area. Ohop Mutual may inspect these products upon request. Ohop Mutual has made no implied warranties or representations with regard to these products or energy savings from these installations.

Ohop Mutual Light Company does not guarantee or warrant any of the work performed on your property. Your remedy for breach of any implied warranty of fitness or merchantability is between you and the contractor performing the work. Ohop Mutual Light Company shall not be liable for any consequential damages arising from any defect in the work performed by your contractor. Customer releases and agrees to defend, indemnify and hold harmless Ohop Mutual Light Company, its successors and assigns, and its respective directors, officers, employees and agents of the Company and its successors and assigns (collectively referred to as the “Indemnities”) from any and all claims, losses, costs, liabilities, damages and expenses (including, but not limited to reasonable attorneys’ fees) arising out of or in connection with any performance of any work, the operation of any equipment, or the acts or omissions of the Contractor or any of its suppliers or contractors of any tier, the respective successors and assigns of the Contractor or any such suppliers or contractors, the directors, officers, employees and agents of each of the foregoing, or anyone acting on the Contractor’s behalf in connection with said performance of work or operation of equipment. To the fullest extent permitted by applicable law, the foregoing release, indemnity and hold harmless shall apply to and for the benefit of the indemnities. This section shall not be intended or interpreted to provide any person an independent claim or course of action based upon third party doctrine.
